CANVAS TA/IIT ACCESS REQUEST FORM

THIS FORM IS NOT USED TO HIRE OR OBTAIN APPROVAL FOR A TA. It is only to be used by an instructor to request that a TA (who has been approved and hired by the normal processes) or IIT be given access to Canvas for a particular section of a particular class in a particular quarter. The TA/IIT will be labelled in Canvas as a TA. While this form is made available in Canvas, it is processed by the Academic Programs Office or the Registrar's Office, not the Office of Teaching and Learning. ONLY THE PRIMARY PROFESSOR FOR A COURSE MAY SUBMIT THIS FORM.
