Welcome to SUP Club, H.B.  Membership is FREE to join IF you have completed a LESSON with Paddle Boarding Hawke's Bay.

If you have NOT completed a lesson - please book either a Beginner or Improver lesson - via Paddle Boarding, Hawke's Bay before completing this form. One Form required per person.  .

I declare that myself and any children (under 18) who are under my direct care as their guardian, abide and agree to the following terms and conditions of SUP Club, Hawke's Bay – supported by Paddle Boarding, Hawke's Bay N.Z Ltd (PBHB Ltd):

• I am at least 18 years or over.  I understand that children between the age of 8 years and 17 years must be accompanied with a parent or guardian.
• I, including family members or children in my care can swim at least 50m unaided.
• I agree that as a SUP CLUB member, I/we will wear a suitable foot leash attached to my board and pfd (life vest) at all times during any session with this club.
• I agree to participate in a safe and appropriate manner.  I/we understand that I/we will be asked to leave the group should I/we be deemed unsafe to be part of this group.
• I/we agree to be in good health and mental and physical fitness at the time of signing this form.  I/we will disclosed to Paddle Boarding, Hawke's Bay, N.Z Ltd any matter concerning my/our health and mental and physical fitness of which I/we am aware, that is relevant to PBHB N.Z Ltd’s decision to permit me/us to be part of this club.  I/we do not hold PBHB Ltd responsible for anything that may happen to me/us relating to health.
• I/we are not currently under the influence of drugs (other than prescription medical already detailed) or alcohol, and accept that if the organiser has reason to suspect that I/us are intoxicated or under the influence of drugs or alcohol before or during a club paddle meeting, I/we will be instructed not to continue the activity as part of the SUP group.
• I will not leave my SUP Club during an organised paddle.  If I/we do go on our own, we will advise the organiser. I understand that I/we will be responsible for our own actions and hold no responsibility to PBHB Ltd should anything happen to myself or family members I am responsible for if I choose to leave the group.
• I understand that I/we are fully responsible for the security of personal possessions during participation in any on-water session.
• I understand that I/we are responsible for any damage or loss that occurs to PBHB Ltd property or that of any other party, should the damage/loss be due to mine or our own negligence or misuse.
• I will listen carefully to all safety briefings and immediately seek clarification on anything which is not entirely clear. 
• If participating in any fitness sessions, I will seek clarification from a medical profession before proceeding.  I will not hold PBHB Ltd responsible for any injuries incurred, i will exercise within my own limits and stop if pain, strain or injury occurs or if I am unable to do a particular exercise.
• I understand and recognise that Stand Up Paddle Boarding is an adventure activity/exercise, and by nature can never be completely risk or injury free. I release Paddle Boarding, Hawke's Bay, NZ Ltd and its officers, employees, agents, licensees, guides and other representatives from all costs, liability, loss or damage incurred or suffered directly or indirectly during the course of the activities in respect of any personal injury, illness or death or damage to or loss of my property (including clothing), however caused.
• I hereby indemnify them against all liability including liability for their negligence (including negligent rescue operations) and/or recklessness and the negligence and/or recklessness of others) for any and all injury, death, loss or damage arising out of or connected with my participation in Paddle Boarding, Hawke's Bay, N.Z (PBHB) activities.
