This event will be held at the Norwich Campus (20 Conkey Avenue) on March 29th from 8:00am - 12:00pm
To make an appointment, simply click on an open white time slot. Enter your information in the appointment box that appears. Before selecting "Create Appointment" please make sure all of the information is correct. After the appointment is created it cannot be changed or cancelled*. Each participant should only make one appointment. Appointments cannot overlap.
When a time slot is full, a colored block will appear and cannot be changed. You will be sent a confirmation email upon scheduling as well as a reminder one day before your appointment.
Please be sure to bring your insurance card and a photo ID with you to your appointment.
Upon arrival, you will be asked to sign a “Health Screening Results and Consent Form” at the registration station. You will then enter the designated area to undergo the screening with a trained technician, and receive extensive health education.
Please Note: All participants are asked to “fast” (no food or beverages, water only) for 8 hours prior to the screening to ensure the most favorable results. Please continue to take any medications as instructed by your doctor. If you are a diabetic or have another medical condition and are unsure if you can fast, please check with your physician. We do not advise that pregnant women participate in biometric screening. While it is safe to screen during pregnancy, experts believe that pregnancy may cause inaccurate results.
