Please use the schedule below to register for 20-minute meetings with Lisa Peklak, RN from Attentive Health.
These meetings will be held onsite @ Penn Stainless in Jackson Center.
At your meeting, you can review the results of your Personal Health Survey, get support from Lisa, and answers to any questions you may have. This meeting with a Health Coach, along with the completion of your Personal Health Survey & Annual Physical, is due by July 1, 2019 to receive your employer's $5 per week wellness credit beginning August 1, 2019.
