Kentucky Mosaic Premium End of Year Service Schedule

Schedule your End of Year (EOY) appointment on the calendar below. To learn more about what this service includes, click here. Once your EOY reservation is booked, you will have the opportunity to schedule your Start of Year appointment.

NOTE: EOY & SOY are two separate services. If you would like us to complete both processes you will need to book separate appointments.  

All costs incurred for these services are to be paid for by KDE. In the box labeled Purchase Order # please enter the discount code:  KDE  This will ensure that your district is not charged for the service. If you are unavailable during your scheduled time you may be charged the retail rate of $1,295 for the services. Please contact Dale.Colyer@education.ky.gov for additional information if needed. 

1. Choose a Date and Time. 
2. Click on an empty white date and time slot to create a new appointment.
3. Fill in the required information. Please verify that the email address is correct, a confirmation email will be sent to that address. 
4. Please also be certain your Customer ID# is correct.
5. Click the "Create Reservation" button.
