Passport Appointment Calendar

Appointments are required for passport services. White fields are available timeslots.  Passport Appointments include photos.

Bring with you:
Completed passport application DS-11 (available at the Library or at travel.state.gov).  PLEASE DO NOT PRINT APPLICATIONS BACK-TO-BACK.  The federal government will not accept them.
•Current Photo ID (valid U.S. drivers license, naturalization certificate, U.S. military ID, government employee card).  PHOTOCOPY of both sides of ID is required.  If copy is not provided, there will be a charge of $1.
•Proof of US citizenship (previous fully valid passport or a certified birth certificate that bears the signature, seal of the registrar, and parent(s) names.)  ORIGINAL plus PHOTOCOPY of it.  If copy is not provided, there will be a charge of $1.
•One color passport photo 2×2 inches in size. We will take passport photos at a cost of $15 per person effective July 1, 2023.

CHILDREN UNDER THE AGE OF 16 - Both parents are required to be here.  If one parent is not able to be here they will need to fill out a Statement of Consent form (available at travel.state.gov) and have it notarized.  We will also need a photocopy of both sides of the non-attending parent's ID.


REMINDER: Passport fees are payable to the US Department of State by CHECK OR MONEY ORDER ONLY.
Ages 16 and up: $130 (passport book) or $30 (passport card)
Ages under 16: $100 (passport book) or $15 (passport card)

Routine service can take up to 8 weeks from the day the application is received at the Passport Processing Agency to the day a new passport is received. 

Expedited Service – Add $60 for each application. Expedited service (for an additional $60) can take up to 3 weeks from the day the application is received at the Passport Processing Agency to the day a new passport is received.

Execution fees ($35 per person) and photo fees ($15 per person) and are payable to the Lancaster Public Library by cash, money order, or credit card.
