Appointments are required.

If you have tested positive for COVID-19, have symptoms of COVID-19, or have been advised by a medical professional to self-quarantine, please wait to schedule your appointment and call to cancel any scheduled appointments.

To schedule an appointment, click on a  WHITE  space. Both  BLUE  spaces and  GREY  spaces are unavailable.
